There are more differences than graphics and levels.
Ramps and obstacles can be removed or added based on the completion of individual stage goals, and can be easily toggled on and off with the flick of the analog stick. So you can actually edit real Tony Hawk levels! Add and Remove pieces, obstacles, and even gaps.
15-20 goals per level. Once you've had enough messing around and find yourself in need of some structure, you can skate on over to any one of the plot-based characters standing throughout each stage, and ask them for a goal to complete. Once the objective has been taken, you're finally required to do more than just beat a couple of high scores or pull off some simple stunts. This time around you have to complete such complicated tasks as beating an inline skater in a super fast street luge race, bust a series of different tricks Tetris style in a cascading order against the clock, or zoom through the level at high speeds in a quest to collect letters that eventually spell bigger words. Complete a goal successfully, and you'll earn a point to add to your character's statistics, making them a much better athlete while opening more of the game up for you. Oh and as an added bonus, whichever goal you complete with one character will carry over to everyone else, erasing the need to do the same thing over and over again with different skaters.
All new mini-games. I'm not sure exactly what mini-games will be featured in THPS4, but the "known" ones are something to do with Tennis and Basketball.
A lot more gaps, and things to unlock. The level landscape looks a lot better, so it doesn't look you are going around plastic levels. More varitey of bails (so you don't do the same 5 bails over and over).
And of course better graphics, gameplay, and absurdly huge levels. Characters have facial expressions, and clothes move when you skate (and you can actually get scrapes on your elbows and knees; you can actually get banged up).
