although the lines themselves may symbolize something, I think the very reason he's carving himself is because this is his first time in the real world. He is Agent Smith who, earlier on in the movie, copied his information into Bane (the guy he appears to be in the real world). Now, Agent Smith was a program, until Neo destroyed him in the first movie. Throughout Reloaded, you'll hear a lot of things about Exile programs, ones that are no longer needed and/or can not be deleted. Smith is an Agent, he is suppose to be unbeatable, such is how he was programmed.

Neo defeated him, but he did not "die" because there is no way to delete an Agent program (I believe it was either the Oracle, or the Creator who implied this). Now, Smith is "free", he can do what ever he wants, he is no longer a program controlled by the Matrix (notice how the other Agents detest him?). Now that he can roam about in the real world, he now can physically feel things, like a human. I believe, that Smith cuts up his hand for the sensation. He has never feeled pain before, so he's experimenting.

easy enough, he wants to find Neo in the real world. Essentailly, Neo would be easier to kill in the real world than in the matrix.

So how come Neo has "powers" in the real world now? there could be multiple reasons.

For one thing, the matrix, including the real world, could all just be one big matrix. This would allow Neo to be powerful as well in the "real" world. If the Real World is actually the REAL World then, we get another slew of possible answers:He is some kind of magical human being, or, he is, essentially, not a human, and more of an outside-Matrix progam not unlike Agent Smith. After all, the creator does say that Neo is a program of the Matrix, designed to keep inline the .1% of people who could not accept living in the Matrix. Those .1% are the people in Zion.

The Creator also says "this will be the sixth time we've destroyed Zion, we are getting quite good at it" do you have any idea what that means? Zion has been destroyed almost the same way numerous times. How can an actual real life city be destroyed, start over with all the same buildings, peoples, and concepts, and then be destroyed again by the machines?

This could prove that the "Real" World is actually part of the Matrix, its another designed world made by the machines to harness the .1% people who can't except what they think is the real matrix. While all along, they are actually in one big matrix, they just can't see it. And that's not all, Neo also says near the end, when the machines are coming, he can "feel them" much like he can sense and feel things inside the matrix. This shows that Neo is starting to see the "Real" World for what it really is: a bunch of green coding.
