I got it. Spoilers below:







There are two options to deal with Cohen after he tells you to go away. You can either kill him or let him live. Killing him now gives you the key to the music box, but you can also let him live and encounter him again later in the game. (You get certain achievements by not killing him initially that you can't get if you kill him right away)
